Sugar Substitute Reduces Risk for Dental Cavities

Swedish researchers completed a study that found Xylitol, which is a sugar substitute has the ability to reduce bacteria that leads to dental cavities. In order to prevent cavities between teeth a relatively high intake would need to be available in order to reduce the acid production between the teeth. Pernilla Lif Holgerson will present her study at the at Umeå University in Sweden on February 23rd.

Xylitol in previous studies have shown reduction in cavities. You can purchase special chewing gum and candy with Xylitol as an ingredient. Xylitol is a sugar alcohol made sugar xylose also refered to as wood sugar. Xylitol is found naturally in fruits and berries such as pears and strawberries.

A series of studies of children and adolescents measured saliva samples for plaque. The studies found that 3.4 grams of xylitol consistently changed the bacteria levels. I did not however, reduce the amount of lactic acid in plaque. Another study found that a single higher dose of xylitol was able to reduce the acidity in the plaque in between the teeth.

In one study they gave 128 children ranging in age from 7 – 12 chewing gum with xylitol. They were instructed to chew a new stick of this xylitol chewing gum three times a day for a total daily dosage of 6.0 grams in one day. Initially, the amount of plaque and the production of lactic acid was reduced dramatically during the trial period. Over a four week time period, the cavity causing bacteria went down by around one-third. Six months later the values returned to the original levels. There were no side-effects reported from the xylitol.

The study concludes that xylitol can be beneficial and recommends the following:

If a person is at high risk for tooth decay they should supplement with a daily intake of xylitol.

Choose xylitol products that stimulate saliva secretion. They recommend xylitol chewing gum as the best method to introduce xyitol into the mouth.

You would need 6 grams of xylitol per day to cancel the production of acid which in turn will reduce the risk of cavities.

For optimal results, deliver the xylitol three times a day, instead of at one sitting.

The chewing gum or xylitol product should contain as much xylitol as possible and not be mixed with other sugars.

This coating is thick on the posterior aspect of the tongue where millions of gram negative bacteria are seen .The thick coating on the tongue is always associated with bad breath. Even a thin biofilm can make anaerobic condition favorable for bacterial proliferation.

4. Dental caries:
This is a destructive process causing decalcification with destruction of enamel and dentine resulting in cavitisation of the tooth. These are produced mainly by the lactobacilli. Food particles are deposited inside these cavities and are putrefied by the anaerobic bacteria producing bad smell. Normal brushing will not remove the food debris easily and hence they are putrefied completely. Caries are common in schoolgoing children and in those who do not maintain proper oral hygiene .Calcium and vitamin deficiency can also predispose caries.

5. Gingivitis:

Saliva can wet these layers and make an aerobic condition which is unfavorable for the bacteria .Any condition which reduces the production of saliva can increase bacterial activity. Some times the salivary duct is obstructed by stones or tumors. Cancer of the salivary gland is associated with offensive odor. In suppurative parotitis purulant discharge into the mouth causes bad breath.

10. Tonsillitis:
Tonsils are a pair of lymphoid tissue situated in the lateral wall of oropharynx. Inflammation of the tonsil is called tonsillitis. Bad breath is seen in both acute and chronic tonsillitis. Quinsy or peritonsillar abscess can also produce bad breath.

11. Tonsillar plaques and tonsillar fluid:
If bad breath persists even after maintaining proper oral hygiene there is possibility of this condition. Serous fluid secreted from the folds of tonsil is very offensive. Some patients complain that they hawk some cheesy materials from the throat, which are very offensive in nature. These are formed inside the tonsillar crypts which contain thousands of bacteria. In such conditions tonsillectomy gives noticeable relief from bad breath.

12. Pharyngitis and pharyngeal abscess:
Pharynx is a fibro muscular tube which forms the upper part of the digestive and respiratory tract. Inflammation of the pharynx is called pharyngitis, caused mainly by bacteria and viruses. Bad breath is present in pharyngitis along with other signs like cough and throat irritation. Abscesses in the wall of pharynx can also produce offensive discharge of pus in to the throat.

13. Dentures:

17. Diabetes mellitus:
Mostly all diabetic patients suffer from bad breath. Coated tongue, ulcers and coatings in the mouth, increased sugar level in tissues are responsible for halitosis. Bacterial growth in diabetic patient is very faster than non diabetic individuals.

18. Fevers:
Bad breath is common in almost all fevers. Even an acute fever can produce bad breath. Severe bad breath is seen in typhoid .Other infectious diseases like Tuberculosis, AIDS produce bad smell.

19. Fasting and dehydration:
Dry mouth favors bacterial activity. So any condition which produces dryness in the mouth makes the breath offensive. Even though the food particles are known to produce bad breath, fasting can also produce the same. Production of saliva is also reduced during fasting. Chewing and swallowing also helps to keep the mouth clean.

21. Diseases of stomach and esophagus:
Eructation of gas and food produce unpleasant smell. Abnormality in the function of lower sphincter can allow the food to regurgitate upwards causing bad breath. Bad breath is also common in gastritis, gastric ulcer and cancer of stomach.

24. Liver disorders:

Clinical studies indicate that at a neutral pH, products containing 10 percent carbamide peroxide are safe and effective. Some side effects of these products include:

Occasional irritation of the inside cheeks and gums

Hydrogen peroxide-containing whiteners:
Some tooth whitening products are gels containing 15-35 percent hydrogen peroxide. The tooth whitening effects of these products can be enhanced using a light or laser system to obtain more immediate results. The gum tissues should be protected with a neutral gel or shield prior to using these products.

Side effects can include:

Tooth sensitivity
Occasional irritation of the inside of the mouth and gums

Tray-based bleaching systems versus gels:
Peroxide-containing products are available as tray-based systems or as gels that can be applied using a brush or painted on. In tray-based systems, the tooth whitening gel is placed in a tray that fits over your teeth. While you wear the tray (for example, overnight), your teeth are bleached. Over-the-counter products provide one-sized-fits all trays that can cause non-uniform bleaching and possible irritation. A dentist can provide you with a custom-fitted tray for better results.

Over-the-counter paints and gels also can be applied non-uniformly (particularly when brushed or painted on). There may be more potential for irritation of the gums and inside of the mouth with these products, since over-the-counter tooth whitening products don’t tend to come with shields to protect these sensitive areas.

Whitening strips:
Whitening strips typically contain a thin coating of a peroxide gel. Whitening strips are usually applied for 30 minutes twice a day for about two weeks. Although they can produce visible whitening in a few days, strips whiten the surface of teeth and not the enamel between teeth, which can result in uneven whitening.

Whitening toothpastes:
Whitening toothpastes are not bleaching products. Those that bear the ADA Seal of Acceptance remove surface stains by polishing or by using chemicals to bind to the chemicals that cause staining. Whitening toothpastes don’t tend to be as effective as peroxide-containing products, but can be used as maintenance products after a bleaching procedure.

In-office treatment versus over-the-counter tooth whitening
Tooth whitening can be performed in-office by a cosmetic dentist or can be performed at home with an over-the-counter tooth whitening product.

There are advantages to professional treatment, including:

More immediate results: A cosmetic dentist has access and the skill to use more concentrated bleaching products.
Greater safety: A cosmetic dentist will be aware of your dental history and will take steps to protect your gums during the tooth whitening process.
More uniform results: A cosmetic dentist will have the experience to apply a tooth whitening product uniformly for the best results.

• Bone Expansion – the walls of the bone is stretched or widened to accommodate the implant

• Socket Preservation Grafting – a donor bone is inserted into the tooth socket after tooth removal and takes four to six months to completely heal.

Almost any area of the body can be improved or corrected by a plastic surgeon. 1. Breast Augmentation

And with the recent re-approval by the Federal Food and Drug Administration of new silicone breast implants, breast augmentation is expected to continue being pursued by many women.

In 2006, breast augmentation was found to be the number one procedure performed by plastic surgeons. The ASPS reported that 329,000 breast augmentations were done that year. Interestingly, Los Angeles breast reduction is also on the rise.

To briefly describe breast augmentation surgery, it entails a plastic surgeon making incisions under the arm, under the breasts, or under the nipples, depending on the surgical approach selected. Then implants, either saline or silicone, are inserted either beneath the breast tissues or all the way beneath the chest muscles. General anesthesia is normally required, but the surgery is almost always an outpatient procedure.

2. Rhinoplasty
The second most commonly performed surgery is rhinoplasty, which is the reshaping of the nose. Both men and women pursue rhinoplasty. Typical reasons that people cite for changing their noses are lack of satisfaction with the current nose, correction of breathing problems, or reconstruction from injury or birth defect.

During a rhinoplasty procedure, the plastic surgeon makes incisions – sometimes inside the nostrils to reduce scarring – and lifts the nose skin off its underlying structure of bone and cartilage. To reshape the nose, the cartilage that is exposed will be trimmed and sculpted. Then the skin will be returned and refitted to the new shape. A plastic surgeon usually takes one or two hours to complete a rhinoplasty, but more complicated surgeries can take longer. Once again, this is usually an outpatient surgery. General anesthesia is used, or, in some cases, local anesthetic with sedation is sufficient.

3. Liposuction
Liposuction is a versatile technique employed by plastic surgeons that is helpful in reshaping many areas of the body. Liposuction can be used to smooth away extra bulges during a tummy tuck. It can be used in areas of the face and neck, or just about any other area of troubling bulges on the body.

During liposuction, a plastic surgeon injects fluid into the fat cells of the area to be treated. An anesthetic to limit pain and a vessel constricting drug to reduce bleeding are included in the injection. Then, very small incisions are made on the skin and tiny tubes are inserted to suction out the fat.

4. Eyelid Surgery
Also known as blepharoplasty, surgery to the eyelids is used to alleviate drooping or puffy upper and lower eyelids. The plastic benefit achieved is a more youthful appearance. A plastic surgeon will make tiny incisions within the natural folds of the skin to hide scars and lift and rejuvenate the eyelids. This is an outpatient procedure done with local anesthetic and sedation.

5. Tummy Tuck
This plastic procedure with the cute name can produce impressive results. For people who have stretched abdominal skin from multiple pregnancies or a large amount of weight loss, the tummy tuck can smooth the torso. Technically called abdominoplasty, the tummy tuck starts with an incision across the lower abdomen within the bikini or underwear line so the scar will usually be covered. Then the plastic surgeon tightens the abdominal muscles into a smoother and firmer contour. Excess skin is removed and the remaining skin is stitched back into place.

A tummy tuck is a major cosmetic surgery that often calls for general anesthesia. With this procedure, a visible scar along your lower abdomen will be present, unlike other procedures that are able to minimize or hide scars.

Although you have many options for enhancing your appearance with plastic surgery, only a plastic surgeon can properly assess what can be achieved with your body. If you are considering a plastic procedure, think through your motivations carefully and also take recovery time into account.

A silver glint in your smile reveals a great deal about your dental history. When you have a cavity, cosmetic dentists use fillings to support and protect the tooth from further decay. For many years, silver amalgam fillings have been the filling of choice. Recently, however, more and more patients have been choosing composite resin or tooth-colored fillings because of their attractiveness and durability.

Silver vs. Tooth-Colored

Silver amalgam contains approximately 35% silver, traces of tin and zinc, and approximately 50% mercury. Mercury is thought to be toxic to humans, and, in the past, has caused some concern in the dental field. This concern has sparked a controversy over the safety of the metal fillings used in the last two decades.

The American Dental Association (ADA) has deemed the much-relied-upon alloy as harmless; however, the ADA warns that those with allergies or a weakened immune system should avoid it.

However, many dentists remain unconvinced of mercury’s safety, arguing that mercury vapor gradually escapes from the filling endangering the patient.

According to dentists, mercury poisoning has been linked to a number of other health problems, including:

The resin is composed of a material similar to that used in dental bonding, another procedure that supports tooth structure.

Resin fillings also require less removal of the original tooth to make room for the filling and are considered a less radical alteration to the natural tooth.

Aesthetically Pleasing

Rather than making teeth look healthy and pristine, silver fillings can checker them with a patchwork of repairs. Often, a grayish color can leak from the filling to the surrounding tooth, worsening the appearance. What is Gynecomastia?

The word Gynecomastia comes from two Greek words, Gyne meaning Woman and Mastos which means breast. A loose translation would be “woman like breasts”. They are also know as steroid breasts or bitch tits as some people unkindly call them.

Gynecomastia actually describes a condition whereby breast tissue builds up in a male's chest creating what appear to be female breasts. These breasts can be quite tender and sensitive.

The condition Gynecomastia is very common in adolescent boys, however, in about 90% of these cases, the condition disappears naturally as their adolescence follows it’s course.

For the remaining 10% though, it remains and can be quite a social handicap causing significant psychological damage to the patient. The feelings that this condition can generate can be shame, embarrassment and humiliation commonly. A lot of this can be brought about from teasing and jokes made about their breasts by their peers.

Other than the tenderness in the chest which is associated with Gynecomastia, there are no other medical problems with this condition other than the psychological problems that it causes.

For some men and boys who appear to have breast reduction male, may actually have a condition called pseudogynecomastia. This describes the fatty build up on their chests. This is not the same thing as Gynecomastia.

Interestingly, it is possible that this condition may affect only one breast or both.

What causes Gynecomastia?

For most cases of Gynecomastia, there are no identifiable causes for this problem. However, this list contains a lot of what is commonly accepted to cause Gynecomastia.

Gynecomastia can also be caused by changes in the balance of the hormones estrogen and testosterone. This commonly occurs during puberty although can probably happen during other stages of a mans life.

From that list, it is believed that the main causes are obesity, steroid abuse and an imbalance in hormones.

How is Gynecomastia Diagnosed?

The doctor will probably make a physical examination of the breasts and then ask a number of questions regarding your breasts in order to ascertain how long you’ve had them and also what degree of tenderness do you experience.

In order to work out what is causing your Gynecomastia, the doctor will enquire about your medical history as well as any medication that you are taking.

The examination itself should be relatively minor with just a bit of prodding and poking.

It should be noted as well that while male breast cancer is more common than a lot of people think, these lumps in a mans breast are not cancerous, however, as usual, if in doubt, contact your doctor as soon as possible in order to eliminate the possibility.

How is Gynecomastia Treated?

In many cases Gynecomastia requires no treatment whatsoever and will disappear quietly on it’s own.

In other cases, the treatment may depend upon the cause. For example, if the cause is the medicine that you’re taking, then hopefully there is a suitable alternative which will be just as effective but without the side of effect of Gynecomastia. As Gynecomastia is usually less of a problem than the condition that the medicine is treating, this other condition will take priority in your care.

In a lot of cases, the reason for Gynecomastia is obesity. In this case, if the patient loses a degree of their weight through diet and exercise, then this will usually cure the Gynecomastia without the need for any kind of cosmetic surgery.

In a small number of cases, surgery is deemed to be the best option. Usually in these cases, the Gynecomastia is causing the patient a significant degree of psychological problems.

This surgery removes the glandular tissue and/or fat behind the breast in order to create a more masculine looking chest.

The surgery for Gynecomastia can often be conducted under a local anesthetic with some sedation and the patient be treated as an out-patient. However, as with all surgery, whether it is low risk like this or high risk, there is always an element of risk of excessive bleeding, fluid loss and problems associated with the anesthesia. As such, the patient should be healthy and emotionally stable otherwise the risk of surgery is increased. There can also be a degree of scarring with the surgery which should be known before the operation takes place.

As this surgery is usually considered cosmetic, it is often quite difficult to get financing or for the public health bodies to accept you as a patient. However, patients do sometimes get the funding required often after proving the psychological problems that this condition causes.

9. Don't be afraid to ask how many of these procedures this dentist has performed successfully.
