Because dental visits are essential, you should work with your implant dentist to develop a regular visitation program to meet your individual needs and to ensure the optimum health of your restorations and surrounding natural teeth.

If your gums and teeth are not properly cleaned and flossed, periodontal disease can develop. If this condition is left alone, the result can be weakened supporting structures and tooth loss in addition to the loss of your dental implants.

Here are some tips to maintain the health of your dental restorations:
  • At-home care: Brushing and flossing daily is essential so that your dental implants will remain healthy. There are a variety of supplemental products that allow you to clean your implants properly.
  • Toothbrush: It may be in your best interest to get an electric tooth brush for a more complete cleaning. A toothbrush with a bent handle to access those hard-to-reach places can also give you a more thorough cleaning.
  • Brushing technique: If you have a removable prosthesis that attaches to your dental implant, once your remove the prosthesis, be sure to brush around your gums and implant and floss thoroughly. If you have a fixed prosthesis, brush the restoration as you would your normal teeth and floss from the front, sides and back.   • Floss threaders: It is also a good idea to pick up some floss threaders. These are very helpful because they allow you to reach around your implants and floss in hard-to-reach areas. Special types of floss are also available, such as floss with a foam coating that cleans around your gums, implants and prosthetic teeth. Be sure to discuss these options with your dentist.
  • Additional brushes: Small dental brushes known as "proxy brushes," or interdental brushes are designed to clean in between your teeth and dental implants. While it is not meant to replace your other toothbrush or your floss, the special brush can help clean areas that the other tools may miss.
  • Toothpaste: Because toothpastes are designed for natural teeth, they contain fluoride. While this ingredient will not harm your implants, you should be sure to read all ingredients carefully. Toothpastes that contain baking soda or abrasive stain removers and those designed for smokers can remove the glaze on porcelain restorations.
  • Mouthwash: Many mouth rinses on the market are designed to give you a fresh- breath feeling. However, alcohol is used to create this and can lead to dry mouth. If you have the tendency toward a dry mouth, you should use an alcohol-free mouth rinse.
  • Regular dental visits: You should have your teeth professionally cleaned at least every six months. But after having dental implants placed, it may be a good idea to see your dentist every three months to thoroughly clean your implants and ensure that there are not any problems. Your specific dental visit program will be discussed and determined after your implants are placed.

Main differences between metal, composite and porcelain fillings.

Fillings are materials placed in tooth cavities to stop additional damage and avoid having the tooth extracted. The decayed tissue is removed, along with bacteria and their toxic debris, and the filling material replaces it.

Metal Fillings:
Metal fillings are traditional and are made from a mix of about 50% mercury, with smaller proportions of silver, copper, tin and zinc. This mix of metals is packed tightly into the cavity and can last for a long time. Durability and strength are the main advantages.

One disadvantage is that they are highly visible, being dark gray and totally unlike tooth enamel. Many people consider them unsightly. Another disadvantage is that they weaken teeth in several ways:

  • By expanding and contracting in response to hot and cold substances put into the mouth, and thus stressing the tooth tissue around them. This also contributes to eventual fractures
Composite Fillings:
Composite fillings are one of the offerings of modern cosmetic dentists. They are done with dental composite, also called dental bonding, white composite, and dental resin. This is a soft, pliable material that comes in a range of white shades. Your cosmetic dentist will choose a shade to match your natural teeth. It is placed directly into the cleaned-out cavity and then hardened with a curing light. This bonds it to the surrounding tooth structure, and makes it a part of the tooth.

Advantages of composite fillings:
  • Aesthetically pleasing tooth-coloured appearance
  • Done in one visit
  • No sensitivity to hot and cold substances
  • Strengthen the tooth by holding it together
Disadvantages of composite fillings:
  • May not last as long as metal or porcelain fillings
  • Susceptible to discolouration over time
  • The tooth needs to be isolated from saliva when the bonding material is placed, but usually this is managed easily with wads of cotton or a rubber covering
Porcelain Fillings:
Porcelain fillings are another offering of cosmetic dentistry, and a very popular one. They are called indirect restorations because they are custom-made in a dental lab and then cemented to the tooth, rather than being applied directly to the tooth. They are called porcelain inlays and onlays. Porcelain inlays are smaller and sit within the cusps. Onlays extend over one or more cusps. Because they are made in a lab from impressions of your teeth, they fit precisely into the cavity.

Advantages of porcelain fillings are:
  • Tooth-coloured ceramic material with a glossy surface like enamel
  • Invisible as fillings, unlike metal fillings
  • The strengthen the tooth, as they’re bonded to it, unlike metal fillings
  • Stain resistant compared to composite fillings
  • More durable and strong than composite fillings
There are no disadvantages of porcelain inlays and onlays except that they require two visits. They give you a strong tooth surface, they last as long as metal fillings and they look far nicer.

Orthodontics was once reserved for teens and preteens, but today over one third of orthodontic patients are adults. Today there are many options available to adults if they want to improve their smile, and the appearance of their teeth. When the straightening of the teeth is the main concern then braces, invisible braces, or porcelain veneers may be suitable for adults. Making a determination on which of these approaches will be the best choice depends on the severity of the problem. The degree of misalignment for teeth that can be effective treated with these braces is slight to moderate.

Their choice of treatment will depend on their personal needs, and the severity of the misalignment. If the team finds it necessary to build up the bone before the implant surgery, the patient will undergo a bone grafting procedure first. Bone grafting is a common procedure for patients suffering from periodontal disease and for dental implant candidates.

Bone Grafting Sources
Replacing missing bone or adding to existing bone is essential for an implant patient with inadequate bone density. There are several material sources available for the bone grafting procedure. The best source is from the implant patient. Material is collected from other areas within the mouth through a suction apparatus during the drilling process for the implants. Sometimes, bones are taken from outside of the mouth such as the hip. If the surgery involves removing material from the hip, the procedure will be done in a hospital by an orthopedic surgeon.

Another common source is from cadavers. The bone is retrieved from a bone bank that has tested it thoroughly to be sure its disease-free. The procedure is very safe and very effective for patients suffering from bone loss. If natural bone isn’t the right fit, other optional materials include synthetic bone, animal bones, coral and biocompatible polymers.

Bone Grafting Procedures
There are several different bone grafting procedures:

• Sinus Grafting – the donor bone is inserted into the floor of the sinus membrane, which is the roof of the upper jaw. This procedure strengthens the egg shell thin wall of bone left behind after upper teeth are removed

• Block Bone Grafting – a block of bone is removed from one area of the patient and screwed into the area where the implants are to be placed. It takes four to six months for the new bone to integrate with the jaw bone

• Guided Tissue Regeneration – when a dentist creates a surgical hole, the soft gum tissue grows quickly over the hole blocking the bone. A membrane barrier is placed over the surgical hole to prevent unwanted gum growth. The membrane is removed later or absorbed by the body

• Bone Expansion – the walls of the bone is stretched or widened to accommodate the implant

• Socket Preservation Grafting – a donor bone is inserted into the tooth socket after tooth removal and takes four to six months to completely heal.

Inlays

Onlays conserve more tooth structure because their use requires minimal removal of a tooth's surface. Onlays also help patients avoid the eventual need for more extensive treatment with crowns, bridges, or dental implants.

Advantages
Since inlays and onlays are made from durable, tooth-colored porcelain typically, they offer much more enduring and natural-looking results than the metal fillings used in the past. Dentists can securely bond them to the tooth's surface, which adds structural integrity and prevents bacteria from entering and forming cavities.

A silver glint in your smile reveals a great deal about your dental history. When you have a cavity, cosmetic dentists use fillings to support and protect the tooth from further decay. For many years, silver amalgam fillings have been the filling of choice. Recently, however, more and more patients have been choosing composite resin or tooth-colored fillings because of their attractiveness and durability.

Silver vs. Tooth-Colored

Silver amalgam contains approximately 35% silver, traces of tin and zinc, and approximately 50% mercury. Mercury is thought to be toxic to humans, and, in the past, has caused some concern in the dental field. This concern has sparked a controversy over the safety of the metal fillings used in the last two decades.

The American Dental Association (ADA) has deemed the much-relied-upon alloy as harmless; however, the ADA warns that those with allergies or a weakened immune system should avoid it.

However, many dentists remain unconvinced of mercury’s safety, arguing that mercury vapor gradually escapes from the filling endangering the patient.

According to dentists, mercury poisoning has been linked to a number of other health problems, including:

· Reduced excretion of porphyrins, which promote oxygen transport in the blood
· A buildup of mercury in various glands in the body
· Sleep disturbances
· Gastrointestinal problems
· Memory problems
· A possible contributory factor to Multiple Sclerosis, Parkinson’s Disease, and Alzheimer's Disease

Silver amalgam fillings have also been known to expand over time, which can sometimes cause teeth to crack causing discomfort and a need for further dental work.

Resin (tooth-colored) fillings involve none of these health risks. They do not expand or damage the tooth. Instead, they are bonded to the tooth, providing strength and support. The resin is composed of a material similar to that used in dental bonding, another procedure that supports tooth structure.

Resin fillings also require less removal of the original tooth to make room for the filling and are considered a less radical alteration to the natural tooth.

Aesthetically Pleasing

Rather than making teeth look healthy and pristine, silver fillings can checker them with a patchwork of repairs. Often, a grayish color can leak from the filling to the surrounding tooth, worsening the appearance. This is especially noticeable if the cavity affects one of the front teeth. “Resin fillings are becoming the norm,” says Dr. Kevin B. Sands, a Beverly Hills cosmetic dentist, “They are better for you and much better-looking.”

With composite resin fillings, people will not even know that you have fillings. The resin mimics the color and translucency of your teeth. The resulting smile is as white and brilliant as it is strong.

Resin fillings cost a bit more, about $150 to $200 per filling, but often the extra expense is covered by insurance. When it comes to dental health, the least expensive option can become more expensive in the long run. Resin fillings last up to ten years before needing replacement.

If you currently have silver amalgam fillings, replacing them with resin composite fillings is a relatively simple procedure.

The dentist may apply temporary veneers on the prepped teeth for the patient to wear during the veneer production waiting period, which is approximately two weeks.

When the permanent veneers have arrived, the patient will return to the dental office so the veneers can be fitted, trimmed and permanently bonded to the teeth using a special light. Once the dentist has determined that the veneers are a perfect fit, the new teeth will be polished and the patient will be able to immediately enjoy an outstanding smile.

Q: What is a dental implant?

A dental implant is a metallic root of a tooth. It is placed in the bone of your jaw by a dental implant dentist and allowed to heal in the bone for a period of time until the bone-implant union is strong enough to support a prosthetic tooth. The implant is made out of titanium, a metal that is very well tolerated by the human body.

Q: How is it used to replace missing teeth?

A: When you are using a dental implant to replace missing teeth, your cosmetic dentist will first determine if you have enough supporting bone to safely place a dental implant to support a prosthetic tooth. The implant is placed into the bone in a very specific surgical procedure to insure that the implant will not be rejected by your body. In order to place the implant in the bone, it is necessary to cut through the gum tissue. The gum tissue will usually be sewed up either over the implant or around the implant after it is placed into the bone. The implant will then be allowed to heal in the bone for several months until the union is strong enough to support the prosthetic tooth.

Q: What kind of dentist do I need to see?

A: Any dentist can place or restore a dental implant. Dental implants are usually placed by Oral Surgeons, Periodontists, Prosthodontists or General Practitioners. Prosthodontists and General Practitioners will usually be the one's who restore the "teeth". You should start by consulting with your general practitioner or a prosthodontist and determine how much experience and training they have in the general field of Dental Implantology. If you elect to go with one of these practitioners, you can follow their recommendation as to who will actually place your dental implant.

Q: Are dental implants a permanent solution to tooth loss?

A: Yes dental implants are considered to be a permanent solution. There are, however, some caveats... Sometimes the dental implant does not integrate and it will be lost. In those cases, another implant might be placed that would stay there and become permanent. Sometimes, the restoration will have to be repaired or serviced... As a matter of fact, most restorations will require some degree of maintenance and sometimes replacement over a period of time. You should be prepared for future expenses in this regard.

Q: Are there people who cannot have dental implants?

A: Most everyone can have dental implants placed, but there are some exceptions and some cautions... Patients who are uncontrolled diabetics or have severe psychological problems are not good candidates for dental implants. Patients with severe medical compromises are not good candidates for anything involving surgery although the dental implant procedures can normally be carried out under local anesthesia and are therefor less risky than surgical procedures requiring general anesthesia.

Patients missing a large amount of bone are not good candidates, but bone grafts can be used to replace the missing bone and allow the patient to have dental implants. You should speak very carefully to your dentist and find out just how good a candidate you are for dental implants. If there are things that will make you a better candidate, you should understand them and help to make the decision as to whether they will be implemented.

Those people who require significant bone grafting for implant placement should engage the services of a qualified oral surgeon. Many minor bone grafts can be accomplished by any competent dentist placing dental implants.

How to avoid problems with dental implants.

1. Find out who has implants and ask them who did them and how they came out.

2. Find qualified ADA specialists, preferably a Board Certified Prosthodontist , to treat you. This is a very demanding area and specialty training is required.

3. Get a detailed, written treatment plan including all fees and make sure there are no hidden costs.

4. Get a second Consultation to help you evaluate the first one.

5. Ask the dentist for names of patients that he or she has treated and for the names of other dentists that he works with. Call them and find out as much as you can about the type of work this dentist does.

6. Determine the commitment that this dentist has to dentistry and to the area of dental implantology. Does he or she teach? Does he or she publish articles in the scientific dental literature. Is he or she considered an "expert" in this area of treatment.

7. Look for someone who is confident, cautious and thorough in his or her approach to your dental work.

8. Call the state board of dentistry and find out if there are any rulings against this practitioner regarding his dental practice.

9. Don't be afraid to ask how many of these procedures this dentist has performed successfully.

10. Ask the dentist about failures. What happens if an implant fails. What are the alternatives if this treatment plan does not work?
